• This membership card is non-transferable.
• If the information filled in the above is found incorrect, membership card will not be issued.
• To avail the discount, this card should be shown to the vendor/ establishment.
• To confirm the authenticity, the vendor / establishment may ask you any of the details given in this form.
• If any vendor/ establishment do not agree to give the mentioned discount in the UNIDECK website, kindly do not argue with the vendor/establishment, Call our toll free number and inform us.
• If the member is found misusing the membership card his/her card will be terminated.
• The card membership subscription fee is for one year and is non-refundable if the membership is discontinued for whatsoever the reason may be.
• UNIDECK membership card application is subject to approval by UNIDECK Card Management Division.
• UNIDECK Card Management Division has the right to approve or disapprove any application without assigning any reason to the applicant.
